IMPORTANT NOTE:
If you use the paper form, you will not get an update on the status of your order (receipt, delivery time, processing and shipping). To benefit from these options, choose the electronic form.
Step 1: Print and complete the signature authentication form.
Step 2: Calculate fees and prepare your payment (see rates). Make sure the total is accurate so that it does not delay your order. We accept paypal, EMT and credit cards.
For credit card payments, please use the “Pay” button below or fill out the section at the bottom of the paper form.
